Optimize patient care transitions and resource utilization in a premier academic medical center. Lead utilization review, discharge planning, and interdisciplinary coordination for high-acuity patients while ensuring compliance with EPIC documentation standards.

Key Responsibilities:

• Conduct utilization reviews for appropriate care level/coverage
• Coordinate discharge planning and post-acute care transitions
• Document care coordination in EPIC
• Ensure compliance with payer guidelines and regulations
• Collaborate with ED teams for rapid case management
• Analyze resource utilization trends and recommend improvements

Requirements

Qualifications:

• Education: ADN/BSN from an accredited program
• Licensure: Current and active New York State Registered Nurse (RN) license is required.
• Certification: Basic Life Support (BLS) certification is required; candidates must hold a current certification or be willing to obtain recertification prior to employment.
• Experience:
• • Must have Pediatric experience.
  • Minimum of 1.5 years of recent experience (within the last 2 years) in an acute care hospital setting of similar size and complexity.
  • Proven experience in hospital case management within an acute care environment is essential.
• Technical Skills: Proficiency in the EPIC Electronic Medical Records (EMR) system is preferred.
